| The same thing elsewhere | $39–$130 at comparable services (NewlyNamed · HitchSwitch · Easy Name Change — real prices compared) — and services that upload your data to their servers must either collect your SSN there or leave those boxes blank |

Honest note
Every government form is free from official sources, and our free guides link to all of them. The packet saves you the hour of finding, decoding and typing — it doesn't unlock anything secret. We are not a law firm and this is not legal advice.
Common questions
What exactly do I get?
Four documents, generated with your details: (1) your completed Social Security Form SS-5, (2) the correct passport form — DS-5504 or DS-82, auto-selected and completed, (3) a set of personalized notification letters (employer, banks, insurance, landlord and more), and (4) your state's step-by-step instruction sheet. You print, sign, and send.
What does it cost?
Nothing. We ask for your email address before generating the documents — that's the whole price. No account, no card, no trial that turns into a charge.
Why is it free?
Name Change Plan is new, and we'd rather earn your trust than $39. The site is supported by clearly-marked affiliate links — if you'd prefer a mailed kit with prepaid envelopes, for example, we point you to NewlyNamed and may earn a commission. The packet itself has no strings attached.
Is my personal information safe?
Your form details never leave your computer. The wizard runs 100% in your browser — the forms are filled locally on your device and downloaded directly. The only thing we receive is the email address you type in; your name, SSN, passport number and address are never sent anywhere.
What do you do with my email?
We keep it so we can tell you if a form you downloaded is revised (the government updates SS-5 and DS-82 every few years) and, occasionally, share name-change tips. Every email has an unsubscribe link, and the list is never sold or shared.
Aren't the government forms free anyway?
Yes — and we link to every free official form in our guides. What the packet saves you is 30–60 minutes of finding, decoding and typing, plus getting the order right for your state. If you'd rather DIY, our free guides and checklist cover you.
What if my situation is complicated?
The packet covers standard marriage name changes and divorce name restoration (with a decree clause). Court-petition cases — no decree clause, changing a child's name — need a court process we don't handle; the wizard tells you upfront and points you to the right resources.
