What does it actually do?

Five questions (state, marriage or divorce, passport age, whether certified copies are in hand, the date) and it renders an ordered plan: the first step, the five government steps with your state's licensing agency, fee and deadline, the passport form that applies, and a computed deadline date where the state has one. It's the same engine that runs on Name Change Plan's own state guides.

Does it collect my visitors' data?

No. The widget asks for a state and a few yes/no answers — no names, no Social Security or passport numbers, no email. It sets no cookies inside the frame. If a visitor clicks through to get forms filled in, that happens on Name Change Plan in a new tab, and even there the form data never leaves their browser.

Is the information reliable?

Federal steps are verified against ssa.gov and travel.state.gov. State fees and deadlines are compiled from official sources and marked ≈ until re-verified each quarter; every state step links to the official agency page so the visitor can confirm. It is not legal advice, and the widget says so.
